Mohawk Valley Health System is seeking an Inpatient Social Worker to support patients and families across our acute care units. In this role, you will conduct psychosocial assessments, develop discharge and care plans, coordinate community resources, and provide crisis intervention and counseling. You will collaborate with an interdisciplinary team to address social determinants of health, advocate for patient needs, and ensure safe transitions of care. Complete comprehensive psychosocial assessments for hospitalized patients and families.
Develop and implement individualized discharge and care plans.
Coordinate community resources, home care, and support services to ensure safe transitions.
Provide crisis intervention, brief counseling, and emotional support.
Collaborate with physicians, nurses, and the care team on treatment and discharge planning.
Advocate for patient rights and address barriers related to social determinants of health.
Maintain accurate, timely clinical documentation in the electronic health record.
Participate in interdisciplinary rounds and case conferences.
Educate patients and families on available resources, benefits, and coping strategies.
Support hospital policies, quality initiatives, and continuous improvement efforts.

Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) – preferred/required per state regulation
Current state social work license in good standing

Case management or discharge planning experience in an inpatient or hospital setting
