Description
Kaiser Permanente is seeking a Medical Social Worker II to deliver high-quality psychosocial care in a technology-driven healthcare environment. In collaboration with clinicians and IT-enabled care teams, you will conduct comprehensive assessments, provide counseling, coordinate discharge planning, and connect patients with community resources. You will leverage digital tools and data to support care transitions, document interventions in electronic systems, and contribute to quality improvement. This role requires strong clinical judgment, empathy, and the ability to navigate complex medical and social situations in a dynamic, collaborative setting.Provide psychosocial assessments and counseling to patients and families
Develop and implement discharge and care transition plans
Coordinate community resources and referrals for patients
Collaborate with multidisciplinary care teams and IT-enabled workflows
Document assessments, interventions, and outcomes in electronic systems
Support crisis intervention and safety planning as needed
Educate patients on available digital tools and healthcare resources
Participate in quality improvement and data-driven care initiatives
