Description

Kaiser Permanente seeks an LMSW Case Manager for our Healthcare & Medical Services department. In this role, you’ll provide psychosocial assessments, discharge planning, and care coordination for diverse patient populations. You’ll collaborate with physicians, nurses, and community partners to develop and implement individualized care plans, ensure smooth care transitions, and connect patients with essential resources. You’ll maintain accurate documentation, support quality and utilization initiatives, and advocate for patients in line with our culture of innovation, collaboration, and continuous improvement.
Conduct comprehensive psychosocial assessments for assigned patients
Develop and implement individualized care and discharge plans
Coordinate services with physicians, nurses, and interdisciplinary teams
Link patients and families with community resources and support services
Advocate for patient needs across the continuum of care
Maintain timely, accurate EHR documentation and case notes
Monitor patient progress and adjust care plans as needed
Participate in quality improvement and utilization management activities
Provide crisis intervention and brief counseling within scope of practice
Educate patients and families on treatment plans and available resources
